Tibial shaft fractures live next door to compartment syndrome and open fracture complications. Soft-tissue respect and alignment goals drive the night as much as nail choice.
Most closed diaphyseal patterns in adults are candidates for intramedullary nailing when soft tissues allow; not every spiral in a low-energy patient needs the same urgency theater.
Decision-making
Screen for open injury and compartments on every handoff. High-energy and segmental patterns need soft-tissue staging thinking. Acceptable alignment parameters exist for non-op care in select stable patterns — but displacement, unacceptable angulation, and open injuries usually point to operative stabilization.
If you nail, plan reduction: traction, blocking screws, and starting point discipline prevent a well-fixed malunion.
Treatment
- Non-op: long-leg casting / functional bracing for select stable closed fractures with close follow-up
- Operative: reamed IM nail for most adult diaphyseal fractures; plate / ex-fix in select proximal/distal or soft-tissue constrained cases
- Open fractures: antibiotics, debridement, and stabilization per open fracture principles
- Serial compartment exams especially early after injury and after nailing

- Compartment syndrome risk is real — document exams
- Open tibia = antibiotics + debridement urgency
- IM nailing is the workhorse for adult diaphyseal fractures
- Malreduction is a technique failure, not fate
