Tibial plateau fractures are soft-tissue injuries that happen to include bone. The articular surface and the column stability matter — but blisters, swelling, and compartments often decide tonight's operation more than the Schatzker type.
Staged spanning external fixation is not a retreat; it is how you keep the soft tissue alive long enough to reconstruct the joint.
Classification
Schatzker remains exam and communication language (I–VI). Add modern column thinking (medial / lateral / posterior) when planning fixation. High-energy bicondylar and medial-sided patterns deserve special respect for soft tissue and vascular risk.
Decision-making
Ask:
- Is the soft tissue ready for an incision over the planned approach?
- Is there compartment syndrome risk?
- How much articular depression / instability is present?
- Is spanning external fixation needed before definitive ORIF?
Low-energy lateral split-depression patterns in older bone may be candidates for early ORIF when soft tissue is friendly. High-energy patterns usually stage.
Treatment
- Spanning external fixation for length, alignment, and soft-tissue recovery in selected injuries
- CT for preoperative articular planning after spanning or when soft tissue allows
- ORIF restoring articular congruence and column stability; dual approaches when needed
- Meniscal pathology is common — address it when encountered
- Early motion once soft tissue and fixation allow
